Social medical workers are friendly, understanding, and knowledgeable personnel who assist patients and their families with their psychosocial needs and more. They are tasked to review the resident plan of care and to keep assessments updated. Our social services team assists both residents and their families during transitional periods. We provide essential information, manage requests and concerns, help in planning care, and manage discharge planning for each resident.
